The P.A.T.H. program provides adult case management, counseling, and psycho-education workshops to the community of Vallejo who are low income. The focus of the program is linkage to community services and resources. The aim of the program is to improve overall behavioral health outcomes by increasing access to services, education, and social support services.  This program is funded under a 3-year grant, and the pilot year is in Vallejo for the first year. In the future, this program will expand to provide service to low income residents of the Fairfield-Suisun area.

P.A.T.H. has partnered with other community resources in building an extensive referral basis. We offer ‘free’ counseling/case management services that assist with linkage and warm hands to other community agencies, such as: food bank, childcare, education/work force, medical/PCP, and mental health referrals. We also provide psychoeducation workshops that focus on development of life skills, social development, decreasing isolation, mindfulness, art, and other support services. We also offer services in Spanish and have language capabilities. 

We have found adults with complex Behavioral Health conditions have housing and/or transportation challenges that impede access to services. We have also found that the pandemic has caused much isolation, affecting individuals financially, socially, physically, and mentally. As a result, there is a great need for in-person workshops and meetings to address issues and to focus on re-developing one’s mental wellness. Linkage to resources is important, as is the social and emotional development these workshops provide for our served population.
